Most of the beach destinations in Mozambique are reached via a combination of scheduled air flights from Johannesburg or Dar es Salaam to main towns (i.e. Pemba for Quirimbas, Vilanculos for Bazaruto), and small aircraft and/or boat transfers to the lodges.
Many places, especially in the north of the country are still very remote and tourism in Mozambique as a whole is relatively new. This means that there is more room for things to go wrong from time to time but it's normally not serious. Although everything possible is done to minimise the chances, you should be prepared for the odd inconvenience.
Remember that when getting to out-of-the-way islands, you'll often go by boat which may sometimes mean you have to wade to get in and out of it. On choppier seas you can get a bit wet so just don't wear you best designer gear.
